The Illinois State Board of Education requires all students entering the 12th grade to present proof of having received a Meningococcal Conjugate Vaccination (Menactra, Menveo, or MenQuadfi) on or after their 16th birthday.

This requirement must be met prior to October 15, 2023 or your child may be excluded from school.

You can get this vaccine at your family doctor and most chain pharmacies. For those families that may require financial assistance, please contact Aunt Martha’s, the county health department, or the VNA. Well-child immunization appointments are limited and fill quickly, so please make an appointment as soon as possible.

When you go to the appointment, please make sure to get written documentation of the vaccine given, including the date of the shot, signature of the provider, and the provider’s address and phone number stamped on the form.
