Illinois State Requirements for Health Documents:

9th grade students

● State of Illinois Certificate of Child Health Exam dated after 8/19/2022

● Illinois Sports Physical if playing sports

● Dental Exam Due May 15 of the current school year

● Eye Exam if you are new to Illinois

● All documents must be turned into the health office by October 15 or within 30 days of enrollment.

Transfer Students

● State of Illinois Certificate of Child Health Exam (Out of state exams not accepted).

● Illinois Sports Physical if playing sports

● Illinois Dental Exam Due May 15 of the current school year

● Eye Exam if you are new to Illinois

● All documents must be turned into the nurse's office within 30 days of enrollment.

Seniors

● Proof of second Meningococcal vaccine (Menactra, Menveo) - 2 dose requirement

○ 1st dose at age 11 years

○ 2nd dose at age 16 years

● All documents must be turned in by October15 or within 30 days of enrollment
