• Vision and Hearing Screenings

    Vision and hearing screenings are provided for students enrolled in District 204 on a yearly basis at the state-mandated grade levels as determined by the Illinois Department of Public Health. If, at any time, a parent/guardian or teacher expresses concerns about a student’s vision and/or hearing, the student will be screened. Parents may contact the school’s health office to request a screening. The following are the IDPH designated grades for screening.

    Vision Screenings:

    • Preschool, Kindergarten*, Second, and Eighth grades
    • All students who receive Special Education services
    • Students who have transferred from out of state
    • Students who have transferred from another district and have not been screened
    • Students in the foster care system

    *Students who are in kindergarten and have received the mandatory eye examination by a physician will not be screened per the Illinois Department of Public Health Protocol.

    Hearing Screenings

    • Preschool, Kindergarten, First, Second, and Third Grades
    • All students who receive Special Education services
    • Students who have transferred from out of state
    • Students who have transferred from another district and have not been screened
    • Students in the foster care system
    • Students who have a known or suspected hearing loss

    If a student fails two screenings spaced two weeks apart, a referral form will be sent home for a physician to complete. Please, return the form to the health office as soon as it is completed.

    Scoliosis Screenings

    The Nursing Coordinator can perform Scoliosis screenings per parental request. If you have concerns regarding your child and a possible postural deformity of the spine (i.e., kyphosis, lordosis, scoliosis), please contact the Nursing Coordinator at 630.740.0188 to schedule an appointment.
