Dental Examination Regulations
All Illinois students entering kindergarten, second, sixth, and ninth grades are required to have an oral health examination. A licensed dentist must perform the examination and sign the Proof of Dental Examination Form. Each student in the above grades must present proof of the examination by a dentist prior to May 15 of the school year. The examination must have been completed within the prior eighteen months of the May 15 deadline. Please turn in the examination report to your child’s school health office prior to the deadline.
If you are having difficulty locating a dentist, the DuPage County Health Department Dental Program has dentists who staff children’s dental clinics during the school year. You may contact the Dental Health Program at 630.682.7400. The DuPage County Health Department Smile Squad visits host schools in our district for preventative care during the school year. If you are interested in your child receiving services, contact your school health office to determine the dates and locations of the Smile Squad.
The College of DuPage Dental Hygiene Program also performs low-cost dental exams. Exams can be scheduled by calling 630.942.3250.
